    Moving west on Morris the green light is out in the traffic signal.
    This is a single lamp problem, not a problem with the fixture or control device.

    Lamp is out in one of the traffic lights.
    Not sure now if it was red (I think so) or green, but its the right hand
    fixture as you are traveling north on Patridge heading towards Madison.
    Just tne one lamp, not a problem with the fixture in general.

    Moving south on Main the red lamp is out in the right hand fixture, at the intersection of Morris and Main.
